With Covid-19 cases growing across the country, it's more important now than ever that households have access to telehealth services.

For the seventh episode of the "Why NC Broadband Matters" podcast series, we spoke with Dave Kirby, president of the North Carolina Telehealth Network Association, about the role of telehealth in the healthcare system, both during the pandemic and after it ends. "Why NC Broadband Matters" is created in partnership with NC Broadband Matters, a nonprofit organization working to connect communities across North Carolina to high-quality broadband access.

The pair discuss how hospital closures and limited access to healthcare impacts rural North Carolina communities. Dave touches on some of the research into how broadband can connect underserved areas to remote healthcare providers. Unfortunately, many rural communities don't have adequate Internet access, and the lack of connectivity is a barrier to telehealth. Christopher and Dave talk about the challenges to expanding broadband for telehealth in rural areas but also about the potential cost savings of better healthcare access in the state

“While most of us take a high-speed Internet connection for granted, many living in rural areas feel disconnected,” states North Carolina television station WRAL’s new documentary, “Disconnected,” which first aired on March 19.

The documentary features local officials, healthcare professionals, small business owners, and families from across the state discussing the importance of high-quality broadband access and the struggle to connect rural areas. Though “Disconnected” was recorded before the Covid-19 outbreak forced schools and businesses to close nationally, the ongoing crisis further emphasizes the necessity of getting all North Carolinians connected to affordable, reliable Internet access.

“Disconnected” was created with help from the Institute for Local Self-Reliance, the North Carolina League of Municipalities, and Google Fiber. Watch the documentary below or on the WRAL website.

A Tale of Two Cities

To illustrate the importance of connectivity for everything from education to healthcare, “Disconnected” takes viewers to two small North Carolina towns — one with high-speed Internet access and one without.

Enfield NC

In Enfield, home to 2,300 people, businesses and residents alike struggle to get connected, and town officials face difficulties attracting new employers to the area. Enfield Middle S.T.E.A.M. Academy reports that about 60 percent of students don’t have Internet access at home. WRAL interviews one student’s family, which only has unreliable satellite connectivity. “It’s a lot of running around,” says Lashawnda Silver, the student’s mother. “If I don’t provide it for her, she’s going to lose out.”

Similarly, an Enfield health clinic says that most patients aren’t able to connect at home and even 40 percent of staff lack home broadband access. “It’s a barrier for their healthcare,” explains Mary Downey, Family Nurse Practitioner.

In February, Christopher was in North Carolina at the Institute for Emerging Issues Forum at North Carolina State University. While he was there, he had the opportunity to conduct several interviews with people engaged in research, working with boots on the ground to expand broadband, or advocating for better policy so more people have access to high-quality Internet access. One of the people he spoke with was Danika Tynes, Ph.D., a Senior Research Associate from the Georgia Tech Research Institute.

One of Danika's areas of expertise is telehealth, which continues to expand in relevance and application with new innovations and the expansion of broadband access. During the conversation, Danika discusses some of the results of her research, including the elements that help telehealth efforts succeed. She also discusses how telehealth applies in different environments and how data can be used to improve its applications for patients and healthcare professionals. Danika also shares a personal experience that illustrates how telehealth is actually more ingrained in our daily lives than we realize.

This week, we have another interview that Christopher recorded while he was at the 2019 Broadband Communities Summit in Austin, Texas. Dr. Robert Wack from Westminster, Maryland, where the town is partnering with Ting Internet, sat down for a conversation on telemedicine.

As the United States’ healthcare system continues to degrade, hospitals, doctors, and other caregivers are looking for new and efficient ways to provide better care for their patients. Broadband is a tool that healthcare professionals are already using for preventative care, consultation, and treatment from a distance. Dr. Wack and Christopher discuss some of the innovations within the healthcare industry that use connectivity, data, and human engagement. These approaches reduce costs and help patients by reducing the stress that accompanies unnecessary trips to the emergency room or can identify when a patient requires medical intervention from the security of their home.

Christopher and Dr. Wack also discuss some of the new challenges that accompany these innovations and strategies for bringing these programs to large groups of people, rather than focusing on small populations.

Dr. Wack updates us on the progress of the network deployment in Westminster and discusses the community’s Mid-Atlantic Gigabit Innovation Collaboratory (MAGIC), the nonprofit established to optimize use of the fiber network they began developing in 2014.

Westminster began its FTTP pilot project last summer and interest is high. Brett Lake from the Carroll County Times recently reported the project is growing. According to the story, the Mayor and Common Council voted in late November to spend city capital projects reserve funds to expand the pilot. 

During the engineering phase of the project, officials identified several opportunities to expand the footprint at nominal construction expense. The additional reach will include an industrial park and a residential neighborhood adjacent to Carroll Lutheran Village, one of the original sites. 

The Village is a continuing care facility of single- and multi-family dwellings. Project leaders hope to learn to serve both with the pilot project. Residents also receive healthcare services so the fiber network will facilitate onsite telehealth.

The city will also install a ring for redundancy within the Air Business Center to connect potential business customers. The business center is adjacent to the industrial park that will now be included in the pilot project.

Each location is near the Carroll County Public Network (CCPN). Westminster will take advantage of the proximity and will tap into the CCPN fiber to reduce costs. The entire pilot project is estimated at approximately $650,000.

Gary Davis, CIO of the Carroll County Public Schools and Chairman of the CCPN, spoke with Chris in Episode #43 of the Community Broadband Bits podcast. The network serves government, schools, and libraries across the county.

Near the center of Florida sits Lakeland, the largest city between Orlando and Tampa with 98,000 residents. The area boasts 38 lakes, citrus crops, and a growing healthcare industry. Lakeland also owns a fiber optic network serving education, business, and government. To learn more, we spoke with Paul Meyer, Lakeland Electric City of Lakeland Fiber Optics Supervisor.

The city's municipal electric company, Lakeland Electric, began generating and providing electricity to customers in its service territory in 1904. In the mid 1990s, the utility began replacing older copper connections between substations with fiber-optic cable. Soon after, the Polk County School District asked Lakeland Electric to connect school facilities via the fiber network for video transmissions. By 1997, almost 50 school facilities were connected to each other via using dark fiber provided by Lakeland Electric. In 1994, the District paid $219,582 $84,737 to the utility to design, construct, and install equipment for video connections in four schools. The school received an indefeasible right of use for two fibers for twenty years. over which Verizon delivers data and voice services to the School District on its own lines.

Meyer noted that the fiber project likely cost more than the school paid but the installation gave them the opportunity to expand the network. Further expansion connected the police department, libraries, and water facilities. Over time, the electric utility has incrementally expanded to every building engaged in city business. The network is aerial, using the utility's own poles to mount the fiber.

As Australia rolls out its National Broadband Network (NBN), an open access mostly FTTH network that will connect 90% of the population (with most of the rest connected with high capacity wireless), it is exploring telehealth opportunities:

“Expanding telehealth services to older Australians still living in their own homes will help health professionals identify potential health problems earlier, reduce the need for older Australians to travel to receive treatment and increase access to healthcare services and specialists.”

Australia has recognized that the private sector will not meet the needs of its businesses and residents and is therefore investing in a next-generation open access network and seeking ways to maximize its social benefits.

Israel appears poised to follow Australia's lead. And what is happening in the US? Well, AT&T admits that DSL is dying, has stopped expanding its supposed next-generation product, and is working state legislatures to prevent others from building the needed networks. SNAFU.
